In your client’s folder, create a separate internal folder for pre-authorizations. Then, use the following templates as a guideline:
Make a copy of the templates and put the duplicate in the right folder; please DO NOT directly write in the templates.
Naming Format:
In order to keep everything consistent, please name your Debrief PPTs the following way:

Due Date:
The Debrief PPT is due less than a week of the evaluation date, as the debrief will be scheduled within a week.
The debrief is to have a separate and more private time with the caregivers to state our findings in an organized, professional manner. If it is a pediatric case, it lets us talk to the parents without the child listening or needing attention during the conversation.
If you need a supervisor to provide feedback or comments, please plan and manage your time accordingly with the due date in mind.
Process:
Evaluation is held and completed
CAST schedules a debrief meeting within a week of the evaluation
Debrief PPT is completed before the meeting
Debrief is held and completed, with CAST joining at the end to explain and answer questions regarding:
- Insurance and Client responsibilities
- Clinic PoliciesClinician explains that Progress Check Week is held every 3 months to discuss progress and future goals as well as discharge criteria.
CAST sends Acknowledgement of Policies Form through SimplePractice, confirms that they signed it prior to ending the meeting.
Clinician sends the Debrief PPT to the caregiver afterwards, noting that the report will be sent to them within the next week.
